What We Are Looking For

The ARC Patient Navigator (CADC) provides substance use disorder counseling and recovery support services to individuals participating in an outpatient Addiction Recovery Clinic. Working collaboratively with medical providers, nursing staff, and behavioral health professionals, this role supports patients receiving FDA-approved medications for substance use disorders (e.g., methadone, buprenorphine, naltrexone) alongside counseling and care coordination. The Patient Navigator focuses on patient engagement, treatment planning, and reducing barriers to care through a compassionate, client-centered, and recovery oriented approach.

Schedule: 4 days per week/10 hours per day, Monday - Thursday, 7:00am - 5:30pm | Location: Brier Clinic, San Bernardino, CA

ESSENTIAL FUNCTIONS AND DELIVERABLES

  • Provide individual and group substance use disorder counseling to patients enrolled in the Addiction Recovery Clinic using evidence-based, recovery-oriented approaches.
  • Conduct substance use disorder assessments, intakes, and ongoing clinical evaluations to inform treatment planning and continuity of care.
  • Support patients receiving Addiction recovery services by providing education, engagement, and recovery support in collaboration with medical and nursing providers.
  • Develop, implement, and update individualized treatment and recovery plans in partnership with the interdisciplinary care team.
  • Serve as a patient navigator to identify, reduce, and problem-solve barriers to treatment, supporting patient engagement, retention, and adherence to care plans.
  • Coordinate referrals and linkages to internal services, community-based organizations, and bridge programs to support timely access to care and continuity of treatment.
  • Engage in outreach and relationship-building with community partners, supporting care transitions, warm handoffs, and collaboration across systems of care.
  • Monitor patient progress and response to treatment, providing follow-up, relapse prevention support, and crisis intervention as clinically indicated.
  • Maintain accurate, timely, and compliant clinical documentation in the electronic health record in accordance with DHCS, Title 9, Title 22, HIPAA, and program policies.
  • Participate in interdisciplinary case conferences and care coordination meetings, collaborating with primary care, behavioral health, and specialty providers.
  • Travel to other clinics, offices, or events as necessary; must have a reliable vehicle, valid driver's license, and auto insurance.

QUALIFICATIONS:

  • Education: High School diploma or equivalent, CCAPP/CADC/CDAC is required. Bachelor's degree is preferred.
  • Licensure/Certification: Valid California driver's license, and auto insurance.
  • Experience: 1+ year of substance abuse counseling in an outpatient clinic setting is required. Must be a Certified Drug and Alcohol Counselor.
  • Essential Technical/Motor Skills:The ability to use a computer on a routine basis and complete required documentation. Use of a computer, spread sheets and internet. Ability to transition to Electronic Health Records.
  • Interpersonal Skills: The ability to communicate positively and effectively with all staff in a multidisciplinary environment. Ability to conduct meetings and present in a public forum. Strong interpersonal relations skills. Fluency in English and Spanish preferred. Ability to maintain a professional attitude in difficult situations and communicate effectively. Understands needs of clinic and works well in a team
    environment.
  • Essential Mental Abilities: Must have critical thinking for comprehensive treatment planning, empathy for building therapeutic rapport, cultural competence for engaging diverse populations, emotional intelligence for effective communication, adaptability for navigating complex environments, and strong communication skills for collaboration with interdisciplinary teams.
  • Work Eligibility: Must be legally authorized to work in the United States on a full-time basis. Must not now or in the future require sponsorship for employment visas.
