Job TitleHousing Care ManagerCompany OverviewAnchor Co-Living provides short-term post-hospitalization and recuperative care housing for residents who are medically stable but still need a safe, supportive place to recover and prepare for their next housing step. The residents we serve are often navigating health recovery, housing instability, and limited support systems at the same time. This role is central to making each stay purposeful, coordinated, and well documented.The OpportunityThis is a strong opportunity for someone who is both relationship-oriented and highly organized. You will work directly with residents during a critical transition period, helping them move from hospitalization or acute health events toward greater stability while ensuring services are documented accurately and on time. For the right person, this is a meaningful next step in housing, case management, or care coordination work because it combines direct resident impact with real ownership of outcomes, compliance, and program quality.What You'll DoBuild trust with residents through regular check-ins to understand their needs, goals, and progress during their stay.Coordinate housing-related care activities that help residents stabilize after hospitalization, illness, injury, or other acute health events.Support discharge planning and next-step transitions to housing placements or other appropriate settings.Own daily documentation for all active residents in the electronic health record, ensuring charts are accurate, timely, and aligned with authorized services.Provide housing navigation support when authorized, including housing search assistance, applications, placement steps, and coordination with community resources and housing providers.Track length of stay, maintain audit-ready charts, respond to documentation feedback, and follow established workflows and templates consistently.What Success Looks LikeWithin the first 6 to 12 months, success in this role will look like:Daily housing notes are completed on time for 100% of active residents.Resident charts remain current, organized, and audit-ready, with no rolling documentation backlog.Documentation clearly reflects resident engagement, support provided, housing progress, and authorized services.Length of stay is monitored closely, helping residents transition appropriately and on time.Discharge documentation is completed promptly and accurately, with zero billing delays caused by incomplete or missing documentation.What We're Looking ForWe are looking for someone who is detail-oriented, consistent, and comfortable building rapport with residents during vulnerable moments. Strong written communication, daily follow-through, and the ability to stay organized in a fast-moving environment are essential. Experience in housing, care coordination, case management, social services, or a related field is preferred. Experience with documentation systems or EHR platforms is a plus.
