About the Role

DialySuites is an individualized dialysis treatment center designed with a specific philosophy: that a lower patient-to-staff ratio leads to superior clinical outcomes and patient thriving. We are seeking a Licensed Clinical Social Worker (LCSW) who is dedicated to patient advocacy and comprehensive care coordination.

Unlike high-volume facilities, DialySuites maintains a limited census. This allows our Social Worker the necessary time to provide meaningful psychosocial support, thorough transplant coordination, and proactive resource management without the constraints of a traditional high-volume caseload.

Position Structure

We offer a unique professional structure designed for clinicians who value both career excellence and personal autonomy:

  • 30-Hour Full-Time Status: This position is structured at 30 hours per week. For the qualified candidate, this includes a full comprehensive benefits package.
  • Schedule Flexibility: We provide the autonomy for the Social Worker to manage and coordinate their own weekly schedule.
  • Collaborative Environment: You will work as an integral member of a small, multidisciplinary team focused on a holistic approach to ESRD care.

Responsibilities

  • Conduct thorough patient assessments to evaluate emotional, social, and behavioral health needs related to dialysis treatment and chronic illness management.
  • Develop and implement individualized care plans incorporating evidence-based interventions such as behavioral therapy, motivational interviewing, and crisis intervention techniques.
  • Collaborate with multidisciplinary teams including medical staff, nurses, and case managers to coordinate holistic patient care and ensure compliance with HIPAA regulations and medical documentation standards.
  • Assist patients and their families with navigating healthcare systems, including intake processes, transportation, insurance, hospice care options, assisted living resources, and community support services.
  • Maintain accurate medical records using EMR (Electronic Medical Records) systems while adhering to confidentiality protocols under HIPAA guidelines.

Requirements

  • Valid licensure as a Licensed Clinical Social Worker (LCSW) with relevant state certification.
  • Excellent communication skills for conducting diagnostic evaluations, case management, intake assessments, and facilitating both individual and group therapy sessions.
  • Ability to work effectively within a multidisciplinary team while managing multiple priorities in healthcare environments.
