Job Overview
We are seeking a dedicated and compassionate Assessment Counselor to join our dynamic mental health team. In this vital role, you will conduct ASAM assessments to determine clients’ needs, develop personalized treatment plans, and coordinate care with multidisciplinary teams. Your expertise will help guide individuals through their recovery journey, providing clarity and support during challenging times. This position is on-site at either of our locations to include Galax, Bassett, or Lynchburg Virginia. Must be licensed (LPC, LCSW, etc.) or Master's degree and eligible for licensure in the state of Virginia required.

Duties:

1. Complete individualized assessment based on ASAM Patient Placement Criteria to determine appropriate level of service for individuals with substance use or co-occurring disorders.

2. Develop person-centered service plans to address needs identified in the individualized assessment.

3. Link and coordinate services for clients as needed such as internal referrals for psychiatric services and external referrals as requested.

4. Participate in interdisciplinary team meetings to include Staff Meetings.

5. Work closely with admission team, clinical team, and medical team to ensure requirements for DMAS Regulations are met.

6. Maintain timely completion of required paperwork and documentation per licensure and insurance regulations.

7. Attend supervision as scheduled.

8. Other duties assigned which are consistent with the philosophy and intent of this position.

ADMINISTRATIVE RESPONSIBILITIES:

1. Assure current and accurate documentation of services in accordance with agency and third party payer requirements.

2. Facilitate individual admission to SUD treatment services and required paperwork throughout individual’s episode of care.

3. Proactively work with SUD treatment team to develop improvements for SUD treatment services.

5. Develop a professional relationship with other Sobrius departments, and community partners.

Pay: $63,000.00 - $70,000.00 per year
