Care Coordinator

Vital Home Care is seeking a full-time Bilingual Care Coordinator (Spanish/English). Coordinators ensure that patients receive quality care through effective communication and administrative tasks.

Hours:

• Monday - Friday: 9:00 am - 5:00 pm

Responsibilities :

• Communicate effectively with patients, aides, and vendors
• Ensure cases are properly staffed and scheduled
• Ensure quality services to patients
• Perform data entry tasks

Qualifications :

• A minimum of 1 Year of Office Experience.
• Excellent time management, problem solving & people skills.
• Strong analytical thinking, communication & organization skills.
• Compassion and empathy.
• Strong attention to detail.
• Able to effectively use a computer, training provided for non-Microsoft programs.
• Bilingual is a plus - English and Spanish

Salary based on experience.
