SUMMARY: To ensure quality and safe delivery of Primary Home Care services. To assist in the provision and coordination of all personal attendant services in accordance with Agency philosophy and established standards. And to grow and nurture referral sources through in-person visits, calls, and educational presentations.
Duties and Responsibilities:
- Provides supervisory oversight to Personal Care Attendants under the Primary Home Care department.
- Ensures client services are provided in accordance with task assignments and conform to Agency policy.
- Periodically monitors service is being provided onsite.
- Maintains contact with office support staff to ensure appropriate services provided.
- Meets with clients and family on occasions to ensure client satisfaction.
- Reports any concerns, problems or issues to appropriate Agency staff.
- Grow and nurture referral sources through in-person visits, calls, and educational presentations.
- Coordinate smooth patient transitions from hospitals or SNFs to our program; gather clinical information and confirm service needs.
- Collaborate with the Interdisciplinary Team to support safe, effective care plans.
- Create and maintain marketing and educational materials; ensure all outreach complies with federal, state, and local regulations.
- Represent the agency at health fairs, civic events, and professional meetings.
- Maintain accurate contact logs, market data, and activity reports.
- Provide regular feedback on community needs, program opportunities, staffing implications, and budget considerations.
- Fulfill additional duties as assigned to advance agency goals.
- Performs Primary Home Care Supervisory Visits.

- Fulfill additional performance responsibilities as assigned and necessary.

WHAT YOU’LL BRING
- Fluent Spanish & English—able to present, write, and build rapport in both languages (required).
- Bachelor’s degree or equivalent work experience in healthcare, marketing, or social services.
- Minimum 3 years of success in hospice, home-health, or healthcare sales/marketing.
- Outstanding communication, negotiation, and public-speaking skills.
- Strong business acumen and proficiency with Microsoft Office.
- Valid Texas driver’s license, reliable transportation, and willingness to travel ~50% locally.
- Ability to lift up to 45 lbs., stand/walk for extended periods, and maintain professionalism under stress.
- Genuine compassion for patients with serious illness and their families.
WORKING ENVIRONMENT
- Office, patient homes, hospitals, SNFs, assisted- and independent-living facilities.
- Frequent interaction with patients, families, clinicians, and community partners.
- Potential exposure to infectious conditions; appropriate training and PPE provided.
Qualifications/Experience:
Education:
· Completion of two (2) years at an accredited college or university preferred or may substitute each year of college with supervisory experience at a Health Care facility, Home Care Agency or Community Agency.
Language skills:
· Bilingual is required.
· Basic computer skills preferred.
Licenses:
· Valid State Driver’s License with good driving record.
Physical Requirements:
· Visual/hearing ability sufficient to comprehend written/oral communication.
· Ability to deal effectively with stress.
· Use hands to grip or feel.
· Reach with hands and arms.
· Occasionally required to bend, stoop and kneel.
· Frequently required to stand on one’s feet for a long period of time.

Job Type: Full-time
Pay: $43,000.00 - $47,000.00 per year
