Full-Time Social Work Associate

Reports To: Director of Social Work

Responsibilities:

• Work with clinical social worker to provide social work services at the service units
• Complete psychosocial assessment for patients as indicated
• Formulate and implement the intervention plan for patients
• Provide concrete case management, follow-up and referral service
• Provide entitlements/benefits eligibility counseling and assist with application
• Assist Clinical Social Workers in providing case management service as needed
• Timely services provided to patients based on the Social Work department guideline
• Complete documentations and reports based on the Social Work department guideline
• Attend case conference, agency meetings and staff meetings
• Participate in community outreach and marketing activities
• Perform other duties as assigned

Minimum Qualifications:

• Bachelor's Degree in Social Science, Social Work, Psychology, Sociology or other related field
• At least one year of working experience in social services, health care agency or related field
• Good verbal and written communication skills
• Bilingual in English/Chinese (Mandarin/Cantonese) required

Compensation and Benefits:

• Salary: $50,000.00 - 55,000.00 per year
