Position Summary

We are seeking a dedicated and compassionate Social Worker to join our team. This role is critical in supporting our residents and their families through every stage of the care journey. The Social Worker will lead care planning efforts, provide emotional support, and serve as a key liaison between residents, families, and the interdisciplinary care team.

This position is available as part-time (20-24 Hours week) , depending on candidate availability and experience. This role requires a strong commitment to supporting clients through their challenges while promoting overall well-being and resilience.

Key Responsibilities

  • Lead and coordinate individualized care planning for residents, ensuring plans reflect residents’ needs, preferences, and cognitive abilities
  • Conduct psychosocial assessments and ongoing evaluations for residents with dementia
  • Provide emotional support and counseling to residents and their families, especially during transitions and changes in condition
  • Facilitate and participate in care plan meetings with families and interdisciplinary team members
  • Serve as a primary point of contact for families, addressing concerns, providing updates, and offering guidance
  • Assist with admissions, transitions, and discharge planning when needed
  • Support residents’ quality of life through engagement, advocacy, and personalized interventions
  • Collaborate closely with nursing, activities, and leadership teams to ensure holistic care
  • Help ensure compliance with all state regulations and facility policies
  • Participate in family education and support initiatives, including caregiver support programs
  • Stay informed about community resources, to aid clients in accessing necessary services.

Qualifications

  • Bachelor’s or Master’s degree in Social Work or related field (BSW or MSW preferred)
  • Experience with care planning is required
  • Experience working with residents with dementia is required
  • Strong knowledge of behavioral health practices and principles.
  • Proficient in crisis intervention techniques and motivational interviewing strategies.
  • Strong understanding of geriatric psychosocial needs and dementia-related behaviors
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively in a fast-paced, team-oriented environment
  • Compassionate, patient, and family-centered approach to care
  • Knowledge of Massachusetts long-term care regulations is a plus
  • Commitment to ongoing professional development in the field of social work.
