ORGANIZATION: Weston United Community Renewal Inc. is a not for profit organization located in Harlem, New York. For over 30 years, Weston United has provided housing and supportive services to participants with histories of chronic homelessness, mental illness, and substance abuse. Weston United employs over 100 employees who contribute to a rich and diverse work environment.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Assist consumers with developing or enhancing ADL Skills; conduct at least 2 apartment visits per week to work with consumers on their individualized Treatment Plans; provide money management training.
  • Assist consumers with integrating into the community; provides crisis intervention during acute episodes of emotional distress in order to contain resident regression and ensure safety of residents and staff; provide Self-Advocacy training
  • Complete all chart documentation in a timely manner; monitor and document consumers’ medication intake and ensure proper storage of medication; document in the Communication Log; facilitate Self-Preservation tests
  • Participate in Fire Drills; assist consumers with grocery shopping, purchasing clothes and personal items when required; maintain collateral contacts at least once per month with ancillary services and prescribing Psychiatrists; attend all assigned trainings; may be assigned to work at alternate sites.
  • Remains on duty until relieved; participates in supervision, staff meetings, and In- Service trainings when required on site and off site.
  • Escorts consumers to community treatment and service providers for scheduled appointments or activities and in emergency situations; reports directly to the Program Director.

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. High School Diploma or GED with two-year related experience. Good writing and communication skills, knowledge of computer.
