Client Care Manager – Home Care

Right at Home is seeking a compassionate, organized, and hands-on Client Care Manager to support our clients and caregivers in the field. This role is ideal for someone who enjoys relationship-building, problem-solving, and making a direct impact on quality care.

This is primarily a field-based position involving visits to client homes, caregiver supervisory visits, quality assurance check-ins, and care coordination throughout the community.

Schedule: 30+ hours/week

• Tuesday–Thursday: 4:00 PM – 9:00 PM
• Friday & Saturday: 12:00 PM – 9:00 PM

What You’ll Do:

• Conduct in-home client visits, assessments, and caregiver supervisory visits
• Support and coach caregivers in the field
• Build strong relationships with clients, families, and care partners
• Help ensure care plans are followed and services remain high quality
• Assist with urgent staffing situations and caregiver/client concerns
• Document visits, coaching, and client updates in WellSky and company systems
• Partner with scheduling and office teams to ensure smooth care coordination
• Participate in urgent response situations as needed

What We’re Looking For:

• Home care, healthcare, caregiving, staffing, or healthcare coordination experience preferred
• Strong communication, organization, and problem-solving skills
• Comfortable working independently and traveling throughout the service area
• Calm under pressure and able to multitask in a fast-paced environment
• CNA, PCA, caregiving, or direct care experience is a plus
• Basic computer skills required
• Valid driver’s license and reliable transportation required
