Care Coordinator, Enhancement Care Management Team (ECMT)
Job Description
The ECMT Care Coordinator is part of a dedicated team supporting individuals experiencing homelessness and those transitioning from incarceration back into the community. Care Coordinators provide direct, client-centered case management services, empowering clients to access housing, healthcare, resources, and opportunities that promote long-term stability, wellness, and successful reintegration.
Responsibilities:
Client Assessment and Planning
- Conduct comprehensive assessments of clients’ needs, strengths, risks, and personal goals.
- Collaborate with clients to develop individualized care plans addressing housing, employment, healthcare, behavioral health, transportation, and social support needs.
- Support clients in identifying achievable short- and long-term goals that promote self-sufficiency and stability.
Resource Coordination
- Connect clients with housing resources, mental health services, substance use treatment, medical care, vocational training, job placement programs, public benefits, and other essential community resources.
- Provide guidance and assistance navigating complex healthcare, social service, and criminal justice systems.
- Assist clients with applications, referrals, appointments, and service linkage activities.
Advocacy and Support
- Advocate on behalf of clients to reduce barriers to housing, healthcare access, employment, and community reintegration.
- Offer emotional support, coaching, and crisis intervention as needed.
- Empower clients to make informed decisions regarding their care, recovery, and personal goals.
Goal Monitoring and Follow-Up
- Regularly review and update individualized care plans to ensure services remain relevant and effective.
- Monitor and document client progress toward established goals and outcomes.
- Conduct follow-up meetings, field visits, and ongoing communication with clients and service providers.
Collaboration and Networking
- Collaborate with community partners, healthcare providers, government agencies, correctional systems, and other stakeholders to coordinate care and access resources for clients.
- Participate in multidisciplinary case conferences, team meetings, trainings, and community outreach events.
Data Management and Reporting
- Maintain accurate, timely, and confidential client records, case notes, and documentation in compliance with organizational and regulatory standards.
- Prepare and submit required reports regarding client progress, service utilization, and outcomes.
- Ensure compliance with all applicable documentation, billing, and reporting requirements.
Crisis Management
- Respond appropriately to client crises and emergencies involving housing instability, health concerns, mental health episodes, or safety risks.
- Implement crisis intervention protocols and coordinate emergency or supportive services as needed.
Transportation and Field-Based Services
- This position may require regular travel within the community to meet clients at shelters, housing sites, healthcare facilities, correctional reentry locations, and other service settings.
- Care Coordinators may be required to use their personal vehicle for work-related travel. Employees must maintain a valid driver’s license, reliable transportation, and current automobile insurance in accordance with organizational requirements. Mileage reimbursement may be provided in accordance with company policy.
