Care Coordinator

The Family Counseling Center is seeking a compassionate, organized, and motivated Care Coordinator to support clients in accessing comprehensive behavioral health and healthcare services. In this role, you will work collaboratively with clients, families, providers, and community resources to develop and monitor individualized care plans that address mental health, substance use, physical health, and social service needs.

The ideal candidate is passionate about helping individuals navigate complex systems of care and committed to improving client outcomes through advocacy, coordination, and support.

Qualifications:

  • Bachelor's degree in Social Work, Psychology, Nursing, Public Health, or a related field (Master’s degree preferred).
  • MSW, MHC, or RN preferred
  • Minimum of 2 years of experience in care coordination or a related field.
  • Experience working with individuals with serious mental illness and substance use disorders.
  • Strong understanding of behavioral health, substance use, and social determinants of health.
  • Excellent organizational, communication, and interpersonal skills.
  • Proficient in EHR systems and care management software.
  • Ability to work independently and collaboratively in a team.
  • Commitment to cultural competence, equity, and professional growth.

Experience

  • Minimum of 2 years of experience in care coordination, case management, behavioral health, or healthcare
  • Experience working with individuals with serious mental illness, substance use disorders, or co-occurring disorders preferred

Skills & Abilities

  • Knowledge of behavioral health, substance use disorders, and social determinants of health
  • Strong organizational, communication, and interpersonal skills
  • Ability to work independently and collaboratively in a team environment
  • Experience using EHR systems and care management software
  • Ability to maintain confidentiality and demonstrate professionalism and integrity
  • Commitment to cultural competence, equity, inclusion, and ongoing professional growth
  • Valid driver’s license and acceptable driving record required
  • Ability to successfully pass background checks and Medicaid exclusion screening

Key Responsibilities

Assessment & Care Planning

  • Conduct assessments of clients’ behavioral health, physical health, and social needs
  • Identify barriers to care and assist in developing individualized care plans
  • Monitor client progress and update care plans as needed

Care Coordination & Service Navigation

  • Coordinate access to CCBHC and community-based services
  • Serve as a liaison between clients, healthcare providers, and community resources
  • Support clients with healthcare navigation and appointment coordination
  • Provide transportation assistance for clients when necessary

Client Advocacy & Support

  • Advocate for client needs within healthcare and social service systems
  • Educate clients and families on available services, treatment options, and health management strategies
  • Help clients build self-management skills and achieve treatment goals

Documentation & Compliance

  • Maintain timely and accurate documentation in the electronic health record (EHR) system
  • Ensure compliance with CCBHC standards, agency policies, and state/federal regulations
  • Participate in quality improvement and staff development initiatives

Team Collaboration

  • Work closely with clinicians, case managers, peer specialists, and multidisciplinary teams
  • Attend team meetings, supervision sessions, trainings, and agency events
  • Support the agency’s mission, Compliance Plan, and Continuous Quality Improvement activities
