Access TLC is seeking a full-time Care Coordinator to join our team. The Care Coordinator will be responsible for case management services and conducting regular field visits with patients. The ideal candidate will coordinate care and ensure patients receive the necessary services in a timely and efficient manner.
Access TLC supports patients by arranging community-based and other services that allow recipients to experience an enhanced and enriched quality of life. The right candidate will be eager to learn, compassionate, kind, and a genuine team player.
This is a hybrid position that includes remote work, field visits, and occasional in-person meetings. Employees will be required to visit participants in the community. Paid travel time and mileage reimbursement are provided for all required work-related travel. Orientation will take place at our San Fernando office, and occasional in-person meetings will also be required throughout employment.
Position Responsibilities:
- Conduct assessments, develop care plans, arrange for and monitor service delivery
- Maintain progress notes and case records
- Conduct quality assurance reviews and collect data
- Make visits to patients to establish ongoing eligibility for the program
- Monitor the safety and services identified in the Individual Service Plan
- Address patient concerns and coordinate appropriate support services
Requirements:
- Bachelor’s or Master’s Degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, or Sociology
- Valid California Driver’s License with a satisfactory driving record
- Reliable vehicle insured in accordance with state and/or organizational requirements
- One year of related work experience; internship or volunteer experience is also acceptable
Job Type: Full-time Monday through Friday 8:30 am – 5:00 pm
