Job Summary UCSF Benioff Children’s Hospital Oakland is seeking an experienced Licensed Clinical Social Worker (LCSW) to support pediatric cardiology patients and families in a multidisciplinary healthcare environment. The LCSW will provide psychosocial assessments, crisis intervention, care coordination, trauma-informed support, and community resource linkage for pediatric patients and their caregivers. Key Responsibilities Conduct comprehensive biopsychosocial assessments for pediatric patients and familiesProvide crisis intervention, trauma-informed care, and emotional supportCollaborate with multidisciplinary teams to develop and implement patient care plansSupport patients and families with coping strategies, education, and advocacyCoordinate referrals to community resources, social services, and behavioral health programsAssist families with housing, transportation, insurance, and financial assistance resourcesAddress child protection concerns and coordinate with protective services when necessaryParticipate in treatment planning and continuity of care initiativesDocument patient interactions and care plans accurately in Epic EMRCommunicate effectively with patients, families, physicians, nursing staff, and allied healthcare teamsSupport high-risk pediatric populations in outpatient and hospital-based settings
