Role Overview
We are looking for a motivated Healthcare Community Liaison to help grow our referral network across Los Angeles. This person will build and maintain relationships with physicians, discharge planners, hospitals, skilled nursing facilities, assisted living communities, case managers, and other healthcare partners.
This is a relationship-driven role. The right candidate is proactive, professional, persistent, and able to represent Doe Home Health with credibility and warmth. You will report directly to our Director of Nursing.
Key Responsibilities
- Develop and grow referral relationships with physicians, hospitals, discharge planners, case managers, skilled nursing facilities, assisted living communities, and senior care organizations throughout the Los Angeles area
- Educate referral sources on the full scope of Doe Home Health's services and communicate our clinical differentiators clearly and confidently
- Maintain consistent outreach through in-person visits, calls, emails, and follow-ups
- Coordinate closely with the clinical and intake teams to ensure referrals are handled quickly and professionally
- Track outreach activity, referral sources, and follow-up needs using CRM or tracking tools
- Represent Doe Home Health at community events, healthcare networking events, and senior care conferences
- Stay informed on local market trends, competitor activity, and opportunities for growth
What We're Looking For
- Experience in home health, senior care, healthcare marketing, medical sales, or referral-based business development
- Existing relationships with physicians, discharge planners, hospitals, SNFs, ALFs, or senior care providers in the Los Angeles area is a strong plus
- Comfortable with field marketing, in-person facility visits, and consistent follow-up
- Strong verbal and written communication skills; able to present services clearly to clinical and non-clinical audiences
- Professional, organized, and able to manage a pipeline of referral relationships
- Persistent without being pushy, and responsive with a strong sense of urgency
- Able to work independently and be held accountable to outreach and growth goals
Requirements
- Valid driver's license and reliable transportation for local travel
- Comfortable using basic CRM, spreadsheet, or tracking tools to log activity and results
- Knowledge of Medicare home health eligibility and referral processes is a plus
