JFS is looking for a motivated Care Manager to provide high-quality services to adults and families. The Care Manager is an integral part of the Care Management Department. Care Managers perform professional work assessing client needs, developing, implementing and monitoring service delivery and assistance plans, coordinating and monitoring services with other agencies, maintaining records and files, and preparing reports. The Care Manager is responsible for providing information, support and services to clients and their families in a timely, professionally ethical and compassionate manner. They provide support to the agency and programs in a manner that is culturally sensitive to the service population.

Duties and Responsibilities:

The Care Manager’s responsibilities include but are not limited to the following:

Client Services:

· Addresses intake calls with potential clients and provides accurate and detailed information

· Completing comprehensive assessments, services plans and quarterly reviews.

· Provides comprehensive care management services to clients

· Ongoing collaboration and engagement with community resources available to support and meet the needs of older adults and adults with disabilities, as well as the needs of families and caregivers.

· Maintain a balanced caseload and meet billable expectations.

· Works in partnership with the care management team and interns to meet clients’ needs.

· Participate in on-call rotation

Program Needs:

· Networking and relationship building with community partners to facilitate Care Management services for clients served.

· Promoting JFS care management services to other organizations to increase awareness and obtain new client referrals.

Qualifications:

  • Bachelor’s in Social Work or other Human Services degree required, Master's in Social Work preferred
  • Familiarity with providing comprehensive case management with older adults
  • Familiarity with age-related health issues, dementia, grief and loss and multidimensional caregiving
  • Personal skills needed to be successful as a Care Manager are creativity, flexibility, organizational skills, time management and the ability to work both independently and as a team player. A Care Manager must have strong communication skills and be able to engage with the client and their family.

The ideal candidate will have experience working with seniors. The ability to effectively communicate and serve as a liaison between the client and the families served, and various community agencies is of the utmost importance.
