Discharge Planner

Job Summary
Responsible for providing management and supportive services to patient/clients under our care to ensure safe and effective treatment that addresses all aspects of the patient/client’s life, needs, and issues. Services will generally include, but not limited to, preadmission assessment, coordination of care, ongoing case management, discharge planning, referral development, coordination of follow up services, and general discharge planning coordination.

Duties

• Collaboration with Case management for completion of assessments for patients regarding aftercare placement.
• Complete family calls as directed to discuss discharge plans for the patient.
• Update spreadsheets for discharge planning locations.
• Collaboration in daily clinical meetings for aftercare options for patients.
• Maintain relationships with other treatment centers that come to our facility.
• Collaboration with case management to complete discharge planning forms and discuss with patients. Ensures release and discharge planning are signed by patient.
• Attend all required clinical meetings.
• Attend in-service training and clinical supervision as required.
• Participate in continuous organization wide quality and performance improvement and comply with all quality improvement requirements.
• Document all material patient interactions in EMR.
• Provide proper documentation and active involvement in treatment team meetings.

Education and Experience

• High School Diploma or equivalent.
• Minimum of 2 years working as a patient care specialist, case manager, BHT supervisor, or related position in in behavioral health, substance abuse, or mental health field.

Pay: $70,000.00 - $72,500.00 per year
