WCS is seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our team. In this role, you will provide direct support and counseling to individuals and families, helping them navigate social, emotional, and behavioral challenges while connecting them with appropriate resources.

Job Function:
- Work a day shift schedule, 8:00 AM - 5:00 PM (1-hour lunch), in a remote setting.
- Participate in care planning meetings and represent your discipline as needed.
- Review discipline-specific documentation for quality and address any deficiencies with field staff in accordance with established policies.
- Monitor field staff documentation of interventions and address or document any issues observed.
- Conduct coaching sessions with field staff as needed to support performance and compliance.
- Communicate regularly with discipline-specific field staff to ensure coordinated, continuous care aligned with members' needs and goals.
- Coordinate and authorize appropriate services in collaboration with the interdisciplinary team (IDT), including support with ADLs, IADLs, housing, meals, and transportation.
- Implement care management activities and interventions to help participants achieve and maintain optimal health and functional outcomes.
Job Requirements:
- Graduate of a Master of Social Work program acceptable to the New York State Education Department (NYSED).
- Minimum of two (2) years of administrative experience in a management capacity within a certified home health agency (CHHA), managed care, long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care, nursing home, or diagnostic and treatment clinic setting preferred.
- Customer service experience required.
- Experience with managed long-term care insurance is a plus.
- Minimum of one (1) year of experience working with a frail or elderly population. Candidates with less than one (1) year of experience may be considered if they meet all other requirements and complete appropriate training upon hire.
