Position Summary
The Licensed Professional Counselor (LPC) provides high-quality, patient-centered behavioral health services to individuals and families at Crossroads Family Care. This role delivers assessment, diagnosis, and therapeutic interventions while supporting integrated care within the clinical team. The LPC promotes emotional wellness, crisis stabilization, and continuity of care in alignment with organizational standards and best practices.
Essential Duties & Responsibilities Clinical Care & Counseling Services
- Conduct behavioral health assessments, screenings, and diagnostic evaluations.
- Provide individual, family, and/or group counseling services.
- Develop and implement individualized treatment plans.
- Deliver evidence-based therapeutic interventions.
- Provide crisis intervention and risk assessments as needed.
Care Coordination & Patient Support
- Collaborate with medical, dental, and behavioral health teams to support integrated care.
- Coordinate referrals to internal and external resources.
- Support patients in navigating care plans and accessing services.
- Promote patient engagement and continuity of care. Documentation & Compliance
- Maintain accurate, timely, and complete clinical documentation in the EHR.
- Ensure compliance with HIPAA, HRSA, and applicable licensing standards.
- Follow policies related to confidentiality, safety, and reporting requirements.
- Participate in audits, chart reviews, and compliance activities.
Quality & Team Collaboration
- Participate in quality improvement and performance initiatives.
- Attend team meetings, trainings, and case consultations.
- Support a collaborative, respectful, and patient-centered environment.
- Maintain professional and ethical standards at all times.
Qualifications
Required
- Master’s degree in Counseling or related behavioral health field.
- Current, unrestricted LPC license in the State of Texas.
- Minimum 1–2 years of clinical counseling experience.
- Knowledge of behavioral health practices, assessments, and treatment modalities.
Preferred
- Experience in an FQHC or integrated care setting.
- Experience with EHR systems.
