Program Summary:
This will be the first PHF in San Diego County. The PHF is a 24-hour LPS designated facility that provides short-term inpatient care for adults 18 years and older who are experiencing acute psychiatric or dual diagnosis needs. The treatment philosophy at Exodus PHF is based on the premise that all individuals have the potential to rehabilitate and to make choices about their environments for living, learning, working and socializing. During an average length of stay of 5-7 days, the interdisciplinary treatment team will develop a plan of care in collaboration with the patient to address their specific needs, often focusing on symptom reduction and stabilization. Teams also prepare for discharge to appropriate community resources that can best meet the unique needs of the patient and improvement in overall wellness.
The Peer Counselor functions as part of the treatment team and is generally the first program site contact for clients. They are actively involved in providing direct or indirect care and supervision of the clients under the discretion of the Program Manager.
MINIMUM QUALIFICATIONS:
- Two years experience working with the chronic mentally ill and substance abusers preferred.
- A stable psychiatric condition of a minimum of 12 months and, as applicable, maintenance of continuous sobriety of 24 months or longer.
- Extensive personal and/or professional knowledge of challenges of developing and maintaining a psychiatrically, sober lifestyle. Verbal and written communication skills.
EDUCATION:
- High School Grad/or GED
- Peer Advocate Certificate preferred.
- Valid CA driver's license and good driving record preferred but not necessary.
