POSITION SUMMARY
The Social Worker serves within an Interdisciplinary Team setting to promote the social health of individuals living in Skill Creations, Inc. facilities. The residents have varying degrees of physical and/or intellectual disabilities and medical care needs. The Interdisciplinary team develops a comprehensive plan of care based on specific needs of each individual. The Social Worker acts as a liaison and representative of the individual and their family to address realistic goals, expectations and needs. Case management includes completing annual evaluations, attending meetings, regular contact with individuals and guardians and documenting services provided.
The contracted number of hours per year is 120 (15 bed facilities) and 96 (12 bed facilities)
Pay range is $41-$51 per hour. This is a Contract Only position.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Pre-admission/Admission services (provide counseling to individuals and families; compile initial Social History/Evaluation; complete pre-admission social work evaluation, attend screening/admission meetings)
- Case management and interim services (regular observations/interaction with individuals; complete annual social work evaluation, assist with obtaining consents)
- Assist with establishing guardianship for individuals when appropriate
- Confer with individuals and family to identify their goals and expectations
- Attend Person Center Planning meetings, interim meetings, medication review meetings as appropriate
- Share results of meetings with guardians if not in attendance
- Make contacts with guardian, at least quarterly to update on individuals’ well- being and progress
- Document guardian contacts and social work services provided
- Assist with planning of and attend biannual Family Socials
- Provide supportive counseling as needed for individuals’ adjustment, concerns, etc.
- Assist guardians with agencies such as DSS, courts, insurance, medical services, etc. as needed
- Assist guardian with final arrangement planning
- Assist with discharge planning (counsel family on placement options, prepare Discharge summary to include 30-60-90 day follow up and provide guardian with each update, document placement/discharge services provided
