- Act as the primary point of contact for patients, families, and healthcare providers, ensuring clear communication and timely information exchange
- Coordinate patient intake, discharge planning, and follow-up care to streamline clinical workflows
- Assist with utilization review and utilization management processes to optimize resource use and ensure compliance with healthcare standards
- Support case management activities by tracking patient progress, updating electronic health records (EHR) systems, and facilitating interdisciplinary collaboration
- Maintain thorough documentation of patient interactions, medical records, CPT coding, ICD-9/ICD-10 coding, and other relevant clinical data
- Collaborate with social work teams, behavioral health specialists, and hospice care providers to address complex patient needs
- Ensure adherence to HIPAA regulations by safeguarding patient information during all interactions and documentation processes
Experience
- Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and discharge planning procedures
- Experience working with EMR (Electronic Medical Records) or EHR systems is essential
- Familiarity with managed care processes, utilization review, and case management practices highly desirable
- Excellent communication skills with the ability to handle intake assessments and coordinate multidisciplinary teams effectively
- Ability to thrive in fast-paced environments requiring attention to detail and critical thinking skills
