Schedule: Tower Hill Men's Recovery Home, Sun-Thurs 7am-3pm
Care Coordinator:
The Care Coordinator is responsible for assisting clients to obtain needed services by providing information, referral coordination and follow-up; develop and connect with resources that address both substance use and mental health disorders; coordinate medical and behavioral services offered through health insurance; coordinate MAT and psychiatric services while client is in the program as well as assisting in the aftercare plan.
1. Supervise and monitor the health and safety of clients in the program.
2. Responsible for maintaining the staffing ratio as assigned by the Program Manager; (remaining on-site as directed by supervisor).
3. Attending and participating in weekly clinical and staff meetings.
4. Assist counsellors and intake coordinators in the intake process as assigned
5. Implement aftercare plans in collaboration with team members.
6. Connect clients with basic needs and treatment resources such as housing, transportation, clothing, income, medical services and behavioral services.
7. Complete and monitor referrals for MAT and medication services
8. Collaborate with providers to ensure a multidisciplinary approach to treatment
9. Educate clients in available resources and assist in supporting them to become independent in connecting with resources.
10. Facilitate psychoeducation groups with consultation and oversight from Clinical Director.
11. Participate in client treatment meetings
12. Assist in day-to-day operations of the program as needed.
13. Responsible for the immediate notification of safety/health/legal issues happening in the program (client or programmatic), including completing an Internal or Critical Incident Report form.
14. Responsible for timely calling out to identified supervisor due to inability to cover the shift.
15. Responsible for completing and authorizing weekly timesheets with your signature to accurately reflect hours worked on shift.
16. · Have knowledge and understanding of DPH regulations and Riverbend's Policy and Procedure manuals.
17. Other duties as assigned.
Knowledge and Skill Requirements:
Educational requirements, one must possess one of the following:
· High School diploma required. Bachelor's Degree preferred. At least a year of experience in the field preferred.
· Must become CPI, CPR/First Aid and MAP certified (training/certifications offered internally)
· Demonstrate the knowledge and ability to promote recovery.
· Valid driver’s license with acceptable driving record.
· Must use company vehicles and potentially personal vehicles to transport clients.
· Strong verbal & written communication skills.
This position requires the successful completion of a CORI check.
