Community Liaison - Home Care

We are seeking a dynamic, relationship-driven Community Liaison/Sales Representative to join our growing home care team. This role is perfect for a motivated professional who thrives on building connections, educating the community, and generating meaningful client referrals.

This is a primarily field-based role (75%+ in the North Bay area).

Compensation & Benefits:

• Salary $65,000-$75,000 per year (Base + Bonuses)
• Mileage reimbursement or company provided vehicle

Responsibilities:

• Build and maintain relationships with referral sources including hospitals, rehab centers, senior living communities, case managers, and other healthcare professionals
• Represent our agency at networking events, health fairs, and community forums
• Conduct educational presentations to promote awareness of home care services
• Develop and execute outreach strategies to meet monthly lead and referral goals
• Collaborate with the internal care team to ensure smooth intake processes, including processing intakes and conducting new client interviews
• Track outreach activity and results using CRM and reporting tools
• Maintain consistent communication with leadership regarding territory performance and opportunities

Qualifications/Requirements:

• Proven experience in healthcare marketing, community outreach, or business development (home care, hospice, or senior services preferred)
• Must have at least 2+ years of home care sales experience in the field, combining client assessments, referral source visits, and client onboarding.
• Excellent communication, public speaking, and relationship-building skills
• Self-starter with strong organizational and time management abilities
• Comfortable working independently in the field and traveling throughout the local territory
• Prior experience in home care, senior living, or healthcare referral networks is a plus
• Valid driver’s license (company-provided vehicle available)
