About the Role
We are seeking a dedicated and driven Care Coordinator to join our Remote Team. This role is ideal for someone who thrives in a Care Manager capacity while also empowering their team to succeed. Care Coordinators are the bridge between clients, community resources, and internal support — ensuring no client falls through the cracks.
Key Responsibilities
Act as primary point of contact for assigned clients, ensuring needs are met and progress is tracked.- Support populations of focus, including:
- Pregnant and postpartum individuals
- People experiencing homelessness
- Justice-involved individuals
- Members with complex health and social needs
Conduct regular check-ins with clients to assess needs and provide linkage to resources. - Collaborate with other coordinators, supervisors, and community partners to achieve the best outcomes.
- Document client interactions accurately and in compliance with program standards.
- Balance a caseload while helping team members navigate challenges.
Qualifications
- Must live in San Bernadino County.
Strong interpersonal and organizational skills. - Experience working with vulnerable populations preferred.
- Ability to work independently while supporting team goals.
- Driven, compassionate, and adaptable to client needs.
