- Conduct thorough psychosocial assessments to understand patients' mental health, social, and environmental needs.
- Identify and evaluate patients' strengths, challenges, areas requiring intervention and barriers to effective care.
- Makes recommendations to provider for treatment options and dispositioning.
- Develop and implement individualized treatment plans in collaboration with LCSW/provider patients, families, and the healthcare team.
- Provide individual, group, and family therapy to address psychological, emotional, and social issues.
- Support patients in managing chronic conditions, adjusting to life changes, and addressing psychosocial stressors.
- Provide crisis intervention and support in various clinical situations.
- Assist in care coordination across hospital and outpatient settings to ensure continuity of care.
- Facilitate referrals to community resources, support services, and additional healthcare providers to support patient needs.
- Advocate for patients' needs and rights within the healthcare system and community.
- Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure integrated and comprehensive patient care.
- Participate in care team meetings, case discussions, and discharge/care planning processes.
- Maintain accurate and up-to-date documentation of patient assessments, treatment plans, progress notes, and other required records.
- Adhere to state and federal regulations, as well as organizational policies and procedures, ensuring compliance in all aspects of social work practice.
- Provide education to patients and families about coping strategies, mental health conditions, and available resources.
- Provide emotional support and guidance to help patients and families navigate complex healthcare situations.
- Offer training and support to staff members on social work practices and psychosocial issues.
- May provide clinical supervision to assigned students, residents or unlicensed MSW staff.
- Engage in ongoing professional development and training to stay current with social work best practices and healthcare trends.
- Contribute to quality improvement initiatives and help enhance social work practices within the health system.
- Meet CEU requirements to maintain licensure.
- Perform related duties as assigned.
- Master's Degree in Social Work (MSW) from an accredited institution.
- Minimum of 6 months to 1 year of clinical experience, with experience in hospital, skilled nursing, or outpatient settings preferred.
- Strong understanding of social work practices, ethical standards, and relevant healthcare regulations.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work effectively within a multidisciplinary team and manage a diverse caseload.
- Proficiency with electronic health records (EHR) and documentation standards.
- Ability to handle high-stress situations with professionalism and sensitivity.
- Ability to travel to other worksites within MVHS.
- Knowledge of EPIC.
- Experience in a hospital inpatient or outpatient environment
- Current and valid New York State licensure as a Master Social Worker (LMSW).
