About the Role:

As a Care Manager, you will be a key member of a multidisciplinary team providing community-based case management and care coordination to adults with psychiatric disorders receiving specialty mental health services. Working within a Department of Mental Health (DMH)-funded program, you will play a vital role in ensuring clients receive the support, services, and housing navigation they need to lead healthier lives.

Key Responsibilities:

• Conduct screenings, assessments, and rehabilitation services in both in-person and field settings
• Facilitate groups, provide crisis intervention, and assist with housing navigation
• Manage outpatient DMH documentation and electronic health records (EHR)
• Transport clients as needed based on program requirements
• Ensure timely client transitions through the continuum of care
• Engage and support individuals with serious mental illness (SMI), substance use disorders (SUD), trauma, justice involvement, and chronic health conditions

Minimum Qualifications:

• Passion for working with vulnerable populations facing mental health and housing challenges
• Three (3) years of clinical experience in health sciences-related fields
• At least one (1) year of direct client care experience
• Experience working with high-utilizing, complex clients who engage frequently with the health system
• Strong ability to work independently and collaboratively within a team
• Knowledge of Cal-AIM documentation requirements and EHR platforms (DMH/DHS preferred)
• Proficiency in identifying and utilizing community resources
• Strong verbal and written communication skills
• Valid CA driver’s license and a good driving record
