Summary

We are seeking a Home Health/Hospice Intake Coordinator to join our team at Iron Point Home Health Care, Inc. In this role, you will be responsible for managing the intake process for new patients, ensuring a smooth transition into our care services. Your contributions will be vital in maintaining our patient-first focus and supporting our mission of restoring maximum function and enhancing well-being.

Responsibilities

• Manage the intake process for new patients, including gathering necessary documentation and information.
• Verify insurance eligibility and benefits for incoming patients.
• Processing insurance authorizations with a variety of commercial and private insurances.
• Coordinate with healthcare providers to ensure seamless transitions into home health care services.
• Maintain accurate medical records in compliance with HIPAA regulations.
• Communicate effectively with patients, families, and caregivers regarding services available and intake procedures.
• Assist in the development of patient care plans in collaboration with clinical staff.
• Ensure all data entry is completed accurately and timely within electronic health record systems.

Requirements

• Proven experience in health information management or a related field.
• Strong understanding of insurance verification and authorization processes and medical billing practices.
• Familiarity with ICD-9 coding is preferred but not required.
• Excellent communication skills, both verbal and written.
• Detail-oriented with strong organizational skills to manage multiple tasks efficiently.
• Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality.
