Assistant Manager, Social Work - Full Time - M-F 8AM-4PM - Atlantic Health Overlook Medical Center, NJ

The Assistant Case Management (ACM) Manager plays a pivotal role in working with the CM Manger to oversee and optimizing the case management processes within the hospital setting. This position involves assisting the CM manager in managing a team of case managers, coordinating patient care plans, ensuring efficient utilization of resources, and maintaining compliance with regulatory standards. The ACM Manager collaborates with interdisciplinary teams to facilitate smooth transitions of care, promote patient-centered care practices, and achieve optimal patient outcomes. Responsible for the direction of case manager and social work team at the hospital and fiscal resources of the hospital Care Management program. Assist the CM Manager in operations of the CM functions across the health care continuum, interfacing with professionals from multiple disciplines to facilitate the best possible outcomes in the most resource-appropriate manner.

Include, but are not limited to:

Leadership and Team Management:

  • Works with CM Manager to Supervise and mentor a team of case managers, providing guidance and support in their daily activities.
  • Foster a collaborative and inclusive team environment, promoting professional growth and development among staff members.
  • Participates on assigned medical staff and hospital corporate committees; promotes effective communication between departments, acting as a liaison to facilitate information sharing, collaborative problem solving and adequate provision of support services; fosters and maintains collaborative relationships within the hospital and the Health System, and with external agencies related to quality and performance initiatives as well as day to day operations issues that may arise.

Responsible for establishing appropriate staffing levels and work assignments, developing performance criteria for staff, serving as an expert resource on laws, policies and operational

  • procedures, and ensuring that the operations of the Care Coordination/Social Work department are consistent with organizational standards.

Care Coordination and Discharge Process :

  • Works with CM Manage Coordinate with healthcare providers to develop and implement comprehensive care and discharge plans tailored to each patient's needs. Providing patient choice options per regulatory requirements
  • Ensure timely and appropriate discharge planning, including arranging post-discharge services and support for patients and families.
  • Leading daily surveillance of observation cases and works with PA discussing any barriers to progression of care or discharge.

Interdisciplinary Collaboration:

  • Collaborate with physicians, nurses, social workers, and other healthcare professionals to facilitate interdisciplinary communication and coordination of care. Leads and educates the Case Managers an Social workers to participate in the interdisciplinary forums
  • Participates and leads the team in interdisciplinary rounds and meetings to discuss patient care plans and address complex cases.
  • Actively communicates information to other CM team members and interdisciplinary teams regarding progress or payer issues related to continued hospitalization and post-acute service associated with the patients discharge plan.
  • Refers cases and issues to Physician Advisors or Designees in compliance with department procedures with timely follow up as indicated.

Compliance and Quality Assurance:

  • Works with CM Manage ensure compliance with regulatory requirements, accreditation standards, and hospital policies related to case management.
  • Conduct regular audits, review of reports to drive outcomes and quality assessments to evaluate the effectiveness of case management processes and identify areas for enhancement.
  • Coordinates with the CM RN and/or CMA to identify and complete the process for CMS required patient notices.
  • Works with Utilization Management Committee and understands the need to complete and documents utilization reviews, physician advisor referrals and other communications related to assigned cases in accordance with department policy and procedure.
  • Complies and leads the team to manage the Condition of Code 44 process, CMS required patient notices and other regulatory requirements within the utilization management process.
  • Maintains proficiency in the application of organization selected clinical review criteria sets evidenced through IRR testing.

Staff Development and Training:

  • Works with CM Manager to implement and align with AHS system expectations with training programs, workshops, and continuing education opportunities to enhance the knowledge, skills, and competencies of social work staff.
  • provide mentorship, coaching, and support to new hires and less experienced team members to facilitate their professional growth and development.

Performance Management and Reporting:

  • Assist the CM Manger to maintain performance metrics and dashboards to track key performance indicators related to case management.
  • 20. Assist the CM Manger to Prepare and present reports on departmental performance, utilization trends, and outcomes to hospital leadership and stakeholders.

Continuous Improvement:

  • Assist the CM Manger to enhance case management processes, standard work , workflows, and documentation practices to improve efficiency and effectiveness.
  • Stay abreast of industry trends, best practices, and innovations in case management and healthcare delivery.

Requirement -

Graduate of an accredited school of Social Work

NJ LSW / LCSW Highly preferred

Experience:

Prior leadership and clinical experience in the care and management of hospitalized patients preferred.

Acute care and care coordination/discharge planning experience preferred.

Certified Case Manager -CCM is highly preferred
