The Riverside Premier Rehabilitation and Healing Center is seeking a dedicated and compassionate Social Worker to join our interdisciplinary team. This is an excellent opportunity to work in a large, fast-paced rehabilitation and long-term care environment focused on delivering high-quality resident-centered care.

Our facility offers a dynamic clinical setting with a strong focus on short-term rehabilitation and long-term care. With a large bed capacity and a state-of-the-art rehab program, we provide services seven days a week and foster a collaborative environment that supports both professional growth and exceptional patient outcomes.

Social Worker Position Overview

The Social Worker plays a key role in supporting residents and their families by addressing psychosocial needs, facilitating care planning, and ensuring safe and effective discharge transitions.

Social Worker Key Responsibilities

• Assess residents’ social, emotional, and psychosocial needs
• Develop and implement individualized care plans
• Participate in interdisciplinary care plan meetings
• Coordinate admissions, transfers, and discharge planning
• Assist with securing home care services, durable medical equipment (DME), and community resources
• Provide support and guidance to residents and families throughout their stay
• Collaborate with clinical and administrative teams to ensure continuity of care
• Assist with financial documentation and resident education as needed
• Advocate for resident rights and quality of life

Social Worker Qualifications

• Bachelor’s Degree in Social Work (BSW), Master’s Degree (MSW), or related field preferred
• Prior experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferred
• Strong communication, organizational, and interpersonal skills
• Ability to manage multiple priorities in a fast-paced environment
• Knowledge of discharge planning, community resources, and care coordination
• Familiarity with PCC and MDS 3.0 is a plus
