El Hogar Community Services is seeking (Mental Health Service Coordinators) to meet the needs of our community mental health programs. Each program is unique in its design and the population they serve. CORE- Marigold, Community Justice Support Program, CORE- Mariposa and Sierra Elder Wellness Program all provides services to the adult population. Summary: The Mental Health Service Coordinator is responsible for delivering mental health, substance abuse, and support services consistent with the principles of the Psychosocial Rehabilitation Model to an adult population with mental illness. The Mental Health Service Coordinator provides Behavioral Health recovery-based services, including, but not limited to: assessments, service plan development, case management, housing support, skills training, group facilitation, crisis intervention, and individual and group therapy. The Mental Health Service Coordinator may be asked to carry a caseload of 75+ clients in the field and maintain contact as defined by the Program Contract(s).
Essential Duties and Responsibilities: include the following. Other duties may be assigned.
· Prepares assessments, progress notes, and other clinical documents in keeping with Agency standards as well as HIPAA regulations;
· Conducts assessments, service plan development, housing plan development, case management, psychosocial rehabilitation, crisis intervention, program transfers, and discharges;
· Ensures consumers assigned to present caseload are within County Authorization;
· Generates appropriate electronic chart notes of all consumer services by the County of Sacramento as well as Agency guidelines;
· Provides group counseling and co-occurring skills groups with clients;
· Provides psychosocial rehabilitation and crisis intervention;
· Conducts individual psychotherapy;
· Participates in internal utilization review as well as Agency quality assurance reviews and willingness to participate in QA subcommittee;
· Actively participate positively during internal staff meetings;
· Carries a client caseload and consistently meets established productivity requirements;
· Assess consumers for homelessness, risk of homelessness, or other identified housing needs.
· Provide necessary linkage to resources to address areas about or potentially affecting the client's ability to obtain or maintain housing.
· Completes and submits high-quality case plans and assessments with minimal supervisory oversight;
· Accurately and comprehensively diagnoses consumer's symptoms utilizing the DSM V and is available for diagnostic consultation with non-LPHA peers;
· Actively contributes positively during El Hogar-provided clinical supervision;
· Maintains active BBS registration.
Supervisory Responsibilities:
This position has no supervisory responsibilities.
Minimum Qualifications:
Education and Experience: Minimum qualifications can be met in the following ways:
· High school diploma and 2 year direct Mental Health experience
OR
Associate Degree and 6 year direct Mental Health experience.
OR
· Bachelor's degree and (4) four years of full-time direct mental health field experience.
OR
· Master's degree or PhD and two years of full-time direct care experience in mental health setting.
· Experience working with adults with substance abuse, criminal justice, mental health and homeless population
Computer Skills:
To perform this job successfully, an individual should know about Contact Management systems; must be able to type 35-45 words per minute; Database software; Internet software, Spreadsheet software, and Word Processing software; Ability to assimilate the County rapidly required billing software (Smartcare).
Certificates, Licenses, Registrations:
· BBS registration and eligibility for LPHA or LPHA (waived) per Sacramento County guidelines is (preferred).
· Current, Valid California Driver's License and driving record acceptable to El Hogar and El Hogar's auto insurance carrier. Current proof of personal auto insurance (Required).
· Current valid CPR training.
Compensation placement is based on Sacramento County Classification:
Other qualified Provider (OQP) - $23.00 – High school diploma and 2year or more mental health experience
MHRS- $ 25.00 - Associate Degree & 6 years (FTE) of direct mental health care
OR
Bachelor's degree 4 yrs (FTE) of direct mental health care;
OR
Master's degree & 2 yrs. (FTE) of direct mental health care;
