Chronic Care Coordinator

Job Summary
We are seeking a dedicated and energetic Chronic Care Coordinator to join our healthcare team. • Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients with chronic illnesses.
• Conduct thorough intake assessments to identify patient needs, barriers to care, and appropriate resources.
• Coordinate hospital discharges and facilitate seamless transitions of care through discharge planning and follow-up.
• Utilize electronic health record (EHR) systems and EMR platforms to document patient interactions, treatment plans, and progress notes accurately.
• Provide education on medical terminology, managed care processes, and patient service expectations to both patients and team members.
• Support case management efforts by tracking patient outcomes, conducting follow-ups, and adjusting care strategies as needed.
• Participate in ongoing training related to ICU experience, trauma center protocols, pediatric care (including PICU), and emergency medicine procedures.
• Proven nursing background with knowledge of medical terminology, discharge planning, and intake procedures.
• Experience working within managed care environments and utilization management processes.
• Strong skills in EMR/EHR systems proficiency along with an understanding of HIPAA compliance standards.

Pay: $50,000.00 - $65,000.00 per year
