Overview

We are seeking a high-level Clinical Care Manager to oversee and coordinate concierge in-home care cases.

This role requires someone who can bring structure, anticipate needs, and manage complexity, ensuring every detail of the case is aligned across staffing, clinical care, and communication.

You’ll work closely with leadership to deliver a seamless, elevated client experience in a fast-paced, growth-oriented environment.

Key Responsibilities

• Own and manage client cases from start to finish
• Coordinate and optimize staffing (RNs, LVNs, caregivers) for each case
• Serve as the primary liaison between clients, families, physicians, and care teams
• Oversee appointments, care plans, and ongoing clinical needs
• Monitor changes in condition and adjust care proactively
• Partner with DPCS and DOO on clinical oversight and staffing strategy
• Support and hold field staff accountable to high standards
• Identify gaps and solve problems quickly and effectively

What We’re Looking For

• Active California license (RN, LVN, or NP)
• 2+ years clinical experience (home health, private duty, or acute care preferred)
• Strong organizational, communication, and decision-making skills
• Experience in care coordination, case management, or team oversight
• Ability to manage multiple high-touch cases with attention to detail
• Comfortable operating in a high-expectation, fast-moving environment
• Reliable transportation and valid driver’s license
