As a Hospice Social Worker (MSW), you’ll provide holistic, patient-centered care—addressing emotional, psychosocial, and practical needs while guiding families through end-of-life decisions with empathy and expertise. Your work will ensure dignity, comfort, and support when it matters most.
✨ What You’ll Do
- Complete comprehensive psychosocial assessments within 5 days of hospice admission
- Identify and address emotional, financial, environmental, and community needs
- Provide grief counseling and emotional support to patients and families
- Assist with:
- Advance directives
- Funeral planning
- Discharge planning
- Transitions in level of care
- Collaborate with the Interdisciplinary Group (IDG) to coordinate care plans
- Participate in team meetings and care planning discussions
- Connect families with community resources and support services
- Foster a respectful, compassionate environment that honors patient dignity and privacy
What You Bring
- Master’s Degree in Social Work (MSW) from an accredited program
- At least 1 year of hospice experience (required)
- Strong communication and counseling skills
- Ability to navigate sensitive conversations with empathy and professionalism
- Valid Driver’s License & Auto Insurance (field-based role)
- AHA CPR Certification
- Experience with HomeCare HomeBase (preferred)
