Perinatal Social Worker (Contract, Part-Time) – Escalation, Mental Health & Resource Support

Aldea Health is a perinatal support company that builds a “village” around Medicaid-covered birthing people—local Doras (community advocates), doulas, nurses, and social workers—so families aren’t left to navigate pregnancy and the first year postpartum alone. Our mission is to close dangerous maternal health gaps for women of color in high-disparity communities by turning coverage on paper into real, continuous, community-rooted support in everyday life.

Aldea Health is seeking a part-time Contract Social Worker (up to 10 hours/week at $30-40/hour) to support our perinatal care team for our inital pilot. In this role, you will:

  • Serve as escalation support for community-based doulas and Doras when birthing people face complex social, behavioral health, or mental health needs.
  • Conduct brief mental health evaluations/screenings and help direct birthing people to appropriate mental health resources and services.
  • Help create, maintain, and refine a practical resource list for common issues affecting Medicaid-covered birthing people (e.g., housing, food access, mental health, transportation, benefits navigation).
  • Review intake information for birthing people to proactively identify emerging or potential problems and suggest follow-up actions.
  • Document contacts, escalations, and referrals in a shared CRM and follow basic data-entry and documentation standards.
  • Provide brief consults and warm handoffs to ensure birthing families are effectively connected to appropriate community and clinical resources.
  • Support occasional trainings or consults for the broader Aldea team (including doulas, Doras, and staff) on available community resources and best practices for connecting birthing people to support.

Ideal candidates have:

  • MSW or equivalent social work training and relevant licensure (or in process)
  • Experience working with Medicaid populations and perinatal or maternal health
  • Deep commitment to health equity and culturally responsive care in high-disparity communities
  • Spanish language skills strongly preferred (not required)

Pay: $30.00 - $40.00 per hour

Work Location: Remote
