Responsibilities Include:
- Provide counseling and emotional support to patients and families
- Coordinate community resources and referrals as needed
- Maintain accurate documentation in compliance with regulatory standards
What do I need for this role with Community Care?
- Bachelor's or Master's degree in Social Work (BSW/MSW)
- Current Social Worker license in the state of PA
- Experience in healthcare or related setting preferred
- Strong communication and problem-solving skills
- Driver's License/Vehicle
