What You'll Do

As a Care Coordinator Associate, you’ll be a key connection point for patients, families, and healthcare providers. Your day-to-day work will include:

  • Receiving and processing referrals from hospitals, physicians, and other healthcare partners
  • Working closely with internal teams to determine eligibility and begin the intake process
  • Serving as a friendly, knowledgeable point of contact for patients, families, and healthcare professionals
  • Answering questions, addressing concerns, and guiding patients to the right services and resources
  • Scheduling appointments while balancing provider availability and patient preferences
  • Managing schedule changes, cancellations, and reschedules with care and attention
  • Sharing important patient information with the appropriate care team members
  • Maintaining accurate and organized patient records
  • Providing compassionate, empathetic support throughout the care coordination journey

What We're Looking For

We’re excited to connect with someone who has:

  • High school diploma or equivalent
  • 1+ years of customer service experience required.
  • 1+ years of home healthcare experience preferred.
  • English & Spanish speaker required
