Essential Duties and Responsibilities
· Evaluates in-home care needs for concierge and premium clients
· Addresses emotional needs and concerns
· Coordinates medical services
· Serves as the primary point-of-contact for families and Clients for all clinical matters; conducts and directs meetings and calls with Clients and their families
· Coordinates with the Client Care team to prepare care givers and to ensure that the Plan of Care is being correctly executed
· Based off the needs of the Clients will, at times, need to perform skilled nursing visits during normal business hours and outside of normal business hours
· Provides Client education and teachings as needed
· Monitors and advocates for medical needs
· Enhances social and quality of life activities
· Establishes short-term and long-term plans
· Discusses difficult topics and complex issues
· Ad hoc projects as needed
Qualifications
· Bachelor’s degree in health sciences, social work or nursing preferred
· 2+ years of experience in healthcare or human/social services industry
· Excellent communication abilities (written and oral)
Job Type: Full-time
Pay: $110,000.00 - $115,000.00 per year
