Position: Virtual Care Coordinator

Location: New York, NY 10017

Job Type: Contract

Contract: 13 weeks (with potential extension)

Schedule: 5 Days/Week | 7.5-hour shifts

Guaranteed hours per week: 37.5

Pay: Up to $28/hour


Job Summary

Pride Health is seeking an experienced Virtual Care Coordinator for a contract opportunity in New York, NY. This role supports virtual hospital services through telehealth, remote patient monitoring, and coordination of patient-centered care while collaborating closely with clinical teams and leadership.


Responsibilities

• Monitor patients remotely for safety and clinical needs

• Support telehealth services and virtual patient care programs

• Triage and assign cases for virtual monitoring/observation

• Collaborate with clinical teams, physicians, and IT partners

• Track telehealth performance metrics and identify trends

• Perform quality assurance reviews and workflow improvements

• Provide real-time troubleshooting support for telehealth platforms

• Document patient observations and escalate clinical concerns

• Assist in training staff on telehealth systems and workflows

• Ensure compliance with HIPAA and patient privacy standards

• Maintain accurate records of patient interactions and safety events


Requirements

• Minimum 3 years of direct patient care experience

• Experience with telehealth or remote patient monitoring

• Familiarity with EMR systems (EPIC preferred)

• Strong communication, multitasking, and problem-solving skills

• Experience with quality assurance, data analysis, and workflow development


Preferred Qualifications

• Experience developing or supporting telehealth programs

• Knowledge of healthcare technology platforms and workflows

• Strong organizational and analytical skills

• Ability to collaborate with interdisciplinary teams
