Social Worker

, a community-focused FQHC serving Brooklyn, is seeking a dedicated and compassionate LMSW, LMHC, or LCSW to provide full time behavioral health services. This is an excellent opportunity for a motivated clinician who values quality care, teamwork, and serving a diverse Medicaid population.

Hybrid position:

• 2-3 days in office.
  
  
  

• $60-$70+/session based on experience
  
  
  

• Perform individual and family intakes
• Provide outpatient psychotherapy to children, adolescents, and/or adults
• Conduct individual and family assessments and treatment for assigned cases
• Obtain and secure Releases of Information from patients or authorized representatives
• Complete medical record documentation in a timely manner
• Participate in team meetings and clinical rounds as applicable
• Engage in training, research opportunities, and ongoing professional development (including supervision, workshops, and self-care)
• Collaborate with clinical leadership for supervision and training goals
• Demonstrate understanding of developmental needs across children, adolescents, and adults
• Maintain compliance with LaSante Health policies and procedures
  
  
  

• Licensure as an LMSW/LCSW/LMHC in New York State
• Pediatric provider preferred
• Strong interpersonal skills
