Care Manager (RN or LVN) - Full Time - Direct Hire

Clermont and Tampa locations

Join a team that’s making a difference in patient care! Provenir Healthcare is hiring Care Managers (RN or LVN) to support high-quality, patient-centered care in an outpatient clinic setting.

What You’ll Do

• Coordinate and manage patient care plans
• Collaborate with physicians and care teams to improve outcomes
• Educate and support patients with treatment plans
• Ensure compliance with clinical guidelines and quality standards

What You Bring

• Active Florida RN or LPN license
• RN: Associate’s or Bachelor’s degree required
• LPN: IV Certification (or ability to obtain within 90 days)
• 1+ year of clinical experience (acute care preferred)
• Strong clinical judgment and communication skills

Nice to Have

• HMO or managed care experience
• Knowledge of HIPAA and healthcare regulations
