Unlike many home care organizations, we prioritize efficiency and quality of life by keeping travel local within Ulster County. This thoughtful approach minimizes time on the road and allows you to focus on what matters most—your patients.

This is a Monday through Friday, 9:00 am – 5:00 pm position with rotating weekend coverage. You’ll be equipped with company-issued technology, including a laptop and mobile phone, to simplify documentation and communication. Patient visits are intentionally capped at no more than five per day, allowing you to deliver the highest standard of care without burnout.

Title: Registered Nurse (RN) Home Care Case Manager

Location: Ulster County, NY

Salary: $100,000 to $125,000 + Sign-On Bonus

Responsibilities for Home Care RN Case Manager:

• Deliver high-quality skilled nursing care to patients in the comfort of their homes following hospital discharge
• Perform thorough patient assessments and create personalized, goal-oriented care plans
• Continuously evaluate patient progress and adjust care plans to ensure optimal outcomes
• Provide education to patients and families on medications, chronic conditions, and preventative health strategies
• Accurately document all patient interactions and clinical updates within the EMR system
• Partner closely with physicians, therapists, and interdisciplinary team members to coordinate care
• Connect patients with community resources and supportive services as needed
• Actively participate in case reviews, team meetings, and care coordination discussions

Requirements for Home Care RN Case Manager:

• Associate’s Degree in Nursing required (BSN strongly preferred)
• At least 1 year of experience as a Registered Nurse
• Prior home care experience is a plus, but not required
• Active New York State RN license in good standing
• Valid NYS driver’s license and reliable transportation
• Ability to work independently and confidently in a field-based environment
• Familiarity with electronic medical records is preferred

Pay: $100,000.00 - $125,000.00 per year
