Experteer Overview In this role you provide social work support to patients in their homes, helping families access services and placement options. You act as a clinic-level social work professional, delivering counseling and coordinating community resources to support care at home. You will work within a multidisciplinary team to address patient and family needs, aiming to improve outcomes and quality of life. This position emphasizes compassionate, patient-centered care and effective resource navigation to empower families. Compensation / Benefits • Perform initial patient assessments and document findings • Document assessments, visits, and contacts in the patient chart • Place calls and make home visits as needed, acting as a resource liaison • Provide patients and families with information on community resources • Attend weekly team conferences to coordinate care • Conduct counseling and support for patients and families • Perform other job-related duties as assigned Tasks • Bachelor’s Degree or equivalent (some certifications require Bachelor’s) • Master’s Degree in social work or equivalent education/experience • Three years' experience in a social agency, hospital, or health/welfare setting (preferred) • Licensed Clinical Social Worker (LCSW) in state of practice (preferred) • Valid Driver's License (required) • Knowledge of community resources and care coordination Key requirements •
