Job Summary
The Lead Care Manager is a hybrid role that combines responsibilities of a Recreational Activity Lead and an Enhanced Care Management (ECM) Case Manager. Under the direction and supervision of the Program Manager and Clinical Director, the Lead Care Manager provides direct care, activity coordination, recreational programming, and comprehensive care management services to adults with developmental disabilities and/or complex health needs. This role integrates day program support with community-based care management to ensure person-centered, interdisciplinary, and high-quality services that promote independence, safety, and overall well-being.

Responsibilities

  • Develop, schedule, and lead recreational and social activities aligned with program goals.
  • Plan and coordinate events (e.g., social gatherings, educational activities, community outings).
  • Prepare and maintain activity spaces, equipment, and supplies.
  • Assist clients with activities of daily living (ADLs), including:
  • Eating and meal setup
  • Mobility and ambulation
  • Toileting and personal care
  • Provide behavioral and emotional support, including redirection and reinforcement of care plans.
  • Monitor and report changes in client condition (physical, behavioral, emotional).
  • Maintain a safe, clean, and supportive environment; report hazards as needed.
  • Participate in drills, trainings, and day program operations.
  • Conduct comprehensive assessments of clients’ medical, behavioral, and psychosocial needs.
  • Develop and implement individualized, person-centered care plans with measurable goals.
  • Coordinate services across healthcare providers, behavioral health, and community resources.
  • Conduct outreach in home, community, and clinical settings.
  • Accompany clients to appointments and assist with system navigation when needed.
  • Support care transitions (e.g., hospital discharge, housing changes).
  • Provide health education and promote self-management skills.
  • Maintain communication with family members, caregivers, and authorized representatives.
  • Complete timely and accurate documentation for all care activities.
  • Maintain records in accordance with program standards, billing requirements, and HIPAA regulations.
  • Utilize electronic health records (EHR) and care management systems effectively.
  • Complete required reports, including real-time documentation and assessments.
  • Ensure compliance with organizational, state, and health plan requirements.
  • Participate in quality improvement (QI) initiatives and performance tracking.
  • Conduct chart reviews, audits, and compliance checks.
  • Monitor service quality and identify gaps in care.
  • Support implementation of evidence-based practices.
  • Assist with incident reporting and follow-up actions.
  • Prepare reports and support audits or accreditation processes.
  • Collaborate with interdisciplinary teams and participate in case conferences.
  • Assist with staff training, onboarding, and ongoing education.
  • Support coordination of staffing and program operations.
  • Promote a positive environment with clients, families, and community partners.
  • Foster culturally competent, trauma-informed care practices.

REQUIREMENTS

Education:

  • High School diploma or equivalent required.
  • B.A. or B.S. in Social Work or equivalent education credits in related field preferred.

Background and Experience:

  • At least 1 year of experience in Enhanced Care Management
  • At least 1 year of experience in direct support, caregiving, or related field
  • Experience working with adults with developmental or complex health needs
  • Knowledge of healthcare systems, case management, and community-based services
  • Strong organizational, communication, and problem-solving skills
  • Ability to manage multiple responsibilities and prioritize effectively
  • Experience with EHR or care management systems (preferred)
  • Proficiency in Microsoft Office (Word, Excel, etc.)
  • Ability to work effectively with diverse populations in a culturally competent manner

Pay: $26.00 per hour
