Position Summary:The LCSW will provide psychiatric social work services for adult inpatient units, focusing on treatment planning, discharge planning, individual and group therapy, and care coordination. This role requires strong skills in psychiatric assessment, crisis intervention, and patient/family support, while collaborating with a multi-disciplinary team.Key Responsibilities:Conduct intake and admission screenings for psychiatric patients, including those on 5150/involuntary holds.Perform bio-psychosocial assessments and document patient responses.Participate in the development and revision of individualized treatment plans.Provide individual counseling, group therapy, crisis intervention, and discharge planning.Educate patients and caregivers to facilitate successful discharge and adherence to care plans.Coordinate care with internal and external team members to ensure continuity of care.Conduct needs assessments, manage orders for durable medical equipment (DME), and facilitate utilization review.Document all care delivery, treatment plans, patient education, and discharge summaries in Epic EMR.Support interdisciplinary teams including nursing, pharmacy, and unit secretaries.
