The Care Coordinator serves as the primary point of contact and operational backbone for IMMORTALS members, delivering a high-touch, concierge-level health experience. This role ensures seamless coordination across diagnostics, providers, and personalized longevity protocols, maintaining a frictionless member journey and precise execution of care.

Position Responsibilities:

• Serve as the dedicated point of contact for members, managing all communications and delivering a white-glove experience
• Coordinate complex scheduling across providers, labs, imaging centers, and specialists (often across time zones)
• Manage end-to-end member journeys including onboarding, diagnostics, and ongoing protocol execution
• Track and facilitate personalized longevity protocols in coordination with the medical team
• Maintain accurate and up-to-date health records, test results, and provider documentation
• Coordinate referrals and follow-ups, ensuring seamless information flow across all stakeholders

Basic Qualifications:

• 5+ years of experience in care coordination, concierge medicine, health coaching, or similar high-touch client services
• Strong ability to manage complex scheduling and logistics with high attention to detail
• Exceptional written and verbal communication skills
• Experience working with high-expectation or high-net-worth clients
• Strong organizational and operational execution skills

Preferred Skills:

• Experience in a startup or fast-scaling health/wellness company
• Background in longevity, preventive, or precision medicine
• Familiarity with advanced diagnostics (MRI, DEXA, CGM, biomarker panels, etc.)
• Experience with CRM/EHR systems (Salesforce Health Cloud, Practice Better, etc.)
• Knowledge of HIPAA compliance and health data management
• Project management experience or certification (PMP, CAPM)

Salary Range: $70-90k + equity + benefits
