Healthy Home Social Worker
Location: 2 N Meridian St, Indianapolis, IN 46204 | Duration: 9+ Months

Pay Rate : $25/hr on 1099 all inclusive

Key Responsibilities

Track EBLL cases in Indiana's NBS system to verify local health departments meet mandated timeframes for case management and closure.

Hold weekly meetings with Case Surveillance Program Manager to review caseloads, county challenges, high-risk cases, and trends.

Partner with Data Analyst to analyze testing patterns and identify district risk factors.

Coordinate with Division Program Coordinators for inspections on elevated cases.

Deliver updates and technical assistance to regional contacts on lead code changes and issues.

Conduct periodic family outreach for re-testing or home visits; travel to district counties for health department meetings.

Present at annual public health nurse meetings; review/edit program guidance and grant applications.

Required Qualifications

Proficiency in Microsoft Office, Adobe, and webinar/conference call tools.

Strong organizational skills to manage hundreds of cases across dozens of counties.

Excellent oral and written communication for professionals, families, health staff, and physicians.

Ability to adapt to change, solve complex problems, think critically, and build stakeholder relationships.

Working knowledge of lead/healthy homes regulations (e.g., 410 IAC 29); HIPAA compliance and good judgment with sensitive data.

Comfort with internet research, procedure development, data entry decisions, and independent work.

Highly Desired

2+ years in case management.

2+ years communicating with families, health departments, and physicians.

2+ years in lead/heavy metal regulations and healthy homes.

Bachelor's degree.
