About the Role

We are seeking a compassionate and reliable Social Worker to support participants enrolled in the California Assisted Living Waiver (ALW) Program—a Medi-Cal–funded program that helps low-income seniors and adults with disabilities access assisted living services as an alternative to nursing facility placement.

This is a per diem, fully remote position, ideal for individuals seeking flexible scheduling while making a meaningful impact. The Social Worker will be responsible for conducting required monthly visits and providing ongoing support to ALW participants residing in Residential Care Facilities for the Elderly (RCFEs).

Key Responsibilities

  • Conduct monthly visits (MVs) via secure telehealth platforms in compliance with ALW and DHCS requirements
  • Monitor participant well-being, adjustment to placement, and overall quality of care within the RCFE
  • Identify psychosocial needs, safety concerns, and changes in condition, and report accordingly
  • Provide emotional support, advocacy, and resource coordination for participants and families
  • Collaborate with RNs, care coordinators, facility staff, and family members
  • Ensure timely and accurate completion of all required documentation in accordance with DHCS ALW guidelines
  • Assist with care plan updates and support ongoing ISP coordination as needed
  • Communicate professionally with interdisciplinary team members and external partners

Qualifications

  • Bachelor’s degree in Social Work (BSW) or related field (e.g., human services, psychology, sociology) required
  • Master’s degree in Social Work (MSW) preferred, but not required
  • Active California Social Work license (ASW or LCSW) preferred, but not required
  • Experience in geriatrics, case management, hospice, home health, long-term care, or care coordination preferred
  • Familiarity with Medi-Cal, Assisted Living Waiver (ALW), or similar programs strongly preferred (training available)
  • Strong assessment, documentation, and communication skills
  • Comfortable conducting visits via telehealth
  • Proficient with technology, including Microsoft OneDrive, Teams, Zoom, Excel, and Adobe (PDF review, completion, and e-signatures)
  • Excellent organizational and time-management skills
  • Reliable internet connection and ability to work remotely
