Care Coordinator - Family/Shared Living with I/DD Waiver Agency

Envision provides a variety of services to support individuals with disabilities and has been a trusted resource for families for over 60 years. Our Family Living program is there to assist when a child or an adult with a developmental disability is unable to live with their family of origin. Envision links them with screened, trained and certified foster families for therapeutic foster care, shared living, and respite services. Our Shared Living Providers are managed through the support of a skilled Care Coordinator who coordinates services; supports and supervises care provided by shared living providers; ensures regulatory compliance with applicable regulations to promote safe, stable, and healthy placements. The position also requires timely completion of monthly documentation. Care Coordinators work collaboratively in a fast‑paced environment to promote safe, stable, and healthy placements across the lifespan.

Key Responsibilities

  • Coordinate person‑centered services and supports in collaboration with individuals served, shared living providers, and interdisciplinary partners such as SSA's, physicians, nursing staff, guardians.
  • Oversee implementation of the Ohio Individual Service Plan (OISP); monitor health, safety, progress, and regulatory compliance through routine home visits.
  • Participate in service planning meetings and support medical, educational, legal, and other appointments as needed.
  • Maintain accurate individual records, including Medicaid documentation, funding oversight, emergency drills, and medication monitoring.
  • Build strong, professional relationships with individuals served, families, providers, and community partners using respectful, people‑first communication.
  • Facilitate team meetings, provide individual‑specific training, and support providers with planning, problem‑solving, and ISP implementation.
  • Promote and protect individual rights; identify advocacy needs and help remove barriers to services.
  • Respond to crises by providing de‑escalation support, coordinating with nursing and service teams, ensuring required reporting, and participating in on‑call coverage.
  • Maintain timely, thorough, and person‑centered documentation in compliance with agency and regulatory standards.
  • Provide regulatory oversight of licensed and non-licensed Shared Living Provider homes, including conducting required safety audits and ensuring compliance with DODD guidelines.

Oversight of Shared Living Providers

  • Build trusting, supportive relationships with shared living providers.
  • Provide training on ISPs, individual background information, and medications at placement and as needed.
  • Support providers’ understanding of agency policies, procedures, and regulatory requirements.
  • Provide real‑time coaching, feedback, and guidance.
  • Monitor provider documentation and compliance with DODD and ODJFS standards.
  • Identify opportunities to enhance provider skills, service quality, and outcomes.

MINIMUM JOB REQUIREMENTS

Education & Experience

  • Required Qualifications
    • Bachelor’s Degree in Psychology, Social Work, Special Education, or a related human services field OR 2+ years of relevant experience in the I/DD demonstrating increased responsibility and understanding.
    • At least 3 years of professional experience supporting individuals with developmental disabilities, mental health diagnoses, or co‑occurring developmental and mental health needs.
    • Experience providing direct or coordinated support in acute mental health, hospital, residential treatment, or similar settings.
    • Minimum of 2 years of supervisory or leadership experience, including providing guidance, coaching, or oversight to others.
    • For internal candidates: must have a positive performance history and not be on a current Performance Improvement Plan.
    • Transportation & Licensing (Required)
    • Valid driver’s license with no more than 5 points.
    • Reliable, insured vehicle available for work‑related travel.
    • Ability to meet agency requirements related to driving and transportation.
  • Preferred Qualifications
    • Professional licensure such as LSW, MSW, LISW, or LPCC.
    • Experience in Foster Care, Shared Living, or family‑based service models.
    • Experience working within trauma‑informed systems or family systems.
    • Background as a mental health technician or similar role.
    • Experience with Applied Behavior Analysis (ABA) concepts.
    • Experience completing or supporting Functional Behavior Assessments (FBAs).

Licenses, Certifications & Training Requirements

  • Valid driver’s license and current auto insurance maintained throughout employment.
  • Ability to complete all required agency and program‑specific trainings within established timeframes.
  • Demonstrated competence in required training and regulatory standards.
  • If professionally licensed, the Care Coordinator is encouraged to complete continuing education to maintain licensure.
  • Background check and drug screen required for employment

Schedule & Work Location

  • Schedule: Full-time 40 hours per week, flexible schedule including some evenings and weekends
  • Work Location: 40% Provider homes and community-based meetings; 60% administrative office on West Fork Campus
  • Work Environment: Work occurs in office settings, provider homes, and community locations. May involve exposure to varied noise levels, behaviors, and weather conditions. CPI training provided.
  • Physical Demands: Office‑based tasks with occasional physical demands, including lifting up to 50 lbs and assisting individuals with mobility, transfers, or self‑care as needed. Reasonable accommodations available.
