Job Overview

"Navitas Healthcare, LLC" is a compassionate and experienced Medical Social Worker (MSW) to support patients and families in an acute care hospital setting. The role focuses on psychosocial support, discharge planning, and coordination of care in a fast-paced environment.

Key Responsibilities

• Conduct psychosocial assessments of patients and families
• Develop and implement individualized discharge plans
• Coordinate post-acute care services, referrals, and community resources
• Collaborate with physicians, nurses, and interdisciplinary teams
• Support patients with emotional, social, and financial challenges
• Manage a caseload of approximately 20–23 patients
• Ensure accurate and timely documentation of social work interventions
• Advocate for patient needs within the healthcare system
• Participate in discharge planning rounds and care coordination meetings
• Help reduce barriers to care and improve patient outcomes

Qualifications

• Master’s Degree in Social Work (MSW) required
• Minimum 1 year acute care hospital social work experience required
• Strong knowledge of discharge planning and care coordination
• Excellent communication and interpersonal skills
• Ability to manage high-volume caseloads in a fast-paced environment
• Strong documentation and organizational skills
